Making a Mortgage Payment Protection Insurance Claim

We will then send you a claim form, which you must fill in and return to us as soon as possible together with any further evidence needed.

Unemployment Claims

You will also need to provide the following information:

• Independent evidence that you remain registered with a benefits office as being unemployed and available for work unless exempt from registration.
• Proof that you are still actively looking for work.

Accident & Sickness Claims

You will also need to provide the following information:

• Independent evidence that you are still disabled and that you remain under the regular care and attendance of a doctor or consultant.
• Proof that you are not receiving any form of payment or reward (other than your normal or statutory sick pay).

Please return completed claim forms to the following address, together with any supporting evidence requested within 90 days of the date your accident, sickness or unemployment occurs.
